The Central Ontario Standardbred Association Membership Forms can now be filled out online.
Once filled out and submitted, the COSA Membership Form will automatically be sent to the COSA Office.
Please note: You cannot use Google Chrome as your web browser when filling out the form. Internet Explorer, Safari, Firefox and all other web browsers are compatible. The form is in PDF Format and requires Adobe Acrobat.
Please direct any questions or concerns regarding membership or the medical/dental insurance benefits to the office at 905-854-2672.
