The annual Eligibility Check forms for grooms on the Ontario Harness Horse Association's Life/Health/Dental benefits under OHHA’s Caretakers’ Premium Assistance
program have been mailed.
Grooms on this program must return this form to OHHA by August 20.
Failure to return this form will result in termination of your benefits.
